Housing Support Case Manager Job Description

Churches United for the Homeless (CUFH) provides shelter and housing for men, women, and families experiencing homelessness in the Fargo-Moorhead area. CUFH works to address the root cause of homelessness by increasing incomes and connecting residents to community services with a goal of increasing housing stability and preventing a return to shelter. A coalition of nearly 60 member congregations, Churches United relies heavily on its faith community partners for prayer, donations, and volunteers and to meet its mission of providing shelter and support in a spiritual setting; however, participation in faith-based activities is completely optional for residents.

PURPOSE Assist people who are homeless or at risk to achieve their highest individual potential. The Housing Support Case Manager will provide case management services to those who qualify for HS funding, includes help connecting to resources in the community, employment, and other resources as needed.

Housing Support Case Manager, Moorhead MN Hours: Full-Time (37/wk) Wage: $17-18/hour Reports to: Director of Support Services

Key responsibilities include:

  • Case managing 19 clients
  • Harm Reduction
  • De-escalation Skills
  • Following Case Management plans for clients
  • Case noting
  • Critical thinking skills
  • Networking with other social services in the community
  • Advocating & broking services
  • Involvement on mental health and chemically dependent workgroups

Successful candidates will have:

  • Excellent customer service, communication, organizational, and time-management skills.
  • Knowledge of and sensitivity to individual struggles with homelessness, poverty, substance abuse, mental illness, and criminal records.
  • Preferred- minimum of 2 years’ applicable experience preferably in social service setting with seniors and adults with disabilities to reduce and prevent return to homelessness.
  • Bachelor of Social Work and Licensed Social Worker preferred or higher.
